DAILY LIVING SKILLS TRAINING

SUPERVISION / GUIDANCE / INSTRUCTION / COUNSELING
 provided to develop the client’s skills and independence with regard to:

ANCILLARY CASE MANAGEMENT SERVICES to assist as needed with benefits, vocational support, and integration of services to all areas of the resident’s life. Individual Service Plan with reviews to insure progress toward the stated goal of independent living.

Furnished home provided. Client is responsible for own transportation other than Regional Transit Authority. A locking security chest is provided to each resident for personal items.  Resident is required to maintain a minimum of 20 hours per week in employment, volunteer or vocational program.  Should the resident become unemployed, either through termination or lay-off, reasonable efforts to assist in a finding a new vocational placement will be made.  Independent living remains a primary motivating factor for personal responsibility. Employment is viewed as an integral aspect of community living.  If after 90 days the resident is not contributing to the community through work or day program then individual goals, programming and residential placement will be re-evaluated.

In all cases, the services received from Carol A. Howe (Supportive Living Services) are intended to support the resident’s own efforts to gain independence.  It is expected that application and admittance into this program is evidence of the individual's desire to move toward the goal of independent living and that the resident will carry responsibility for participating in the program as it is written in the annual service plan.  Should the resident change this long-term goal, the support network will be notified and arrangements made for finding a program that more appropriately fits the individual's changing needs.
